Information You Send Us
Our appointment request form asks for your first and last name, phone number, email address, the days and times that work for you, and whether you’re a new or existing patient. The form also records the page you came from and any campaign tags in the link you used to reach us, so we know how people find the site.
When you send the form, our web server emails your request to our front desk. We use it only to call you back and schedule your visit. The form doesn’t ask about your health, and we ask you to leave health details out of it. We’ll talk through what you need by phone.
Information Collected Automatically
Like most websites, our web server keeps standard logs of visits, including IP address, browser type, the page requested, and the time. Our hosting provider uses these logs for security and troubleshooting.
We also use Google Analytics to understand how people use the site: how many visits we get, which pages people read, how they found us, the general area they’re visiting from, the type of device they use, and which buttons they tap, such as the phone number or directions link. Google Analytics uses cookies to do this. It doesn’t log or store IP addresses, and we don’t use it to identify anyone. We never send form entries to Google Analytics, and we’ve turned off its advertising features.

Maps + Online Bill Payment
The map on our site is provided by Google Maps. When the map loads, Google may collect information about your visit under its own privacy policy.
When you choose to pay a bill online, you leave our website for a secure payment page run by our payment processor. Anything you enter there goes to the processor under its own privacy policy. Our website doesn’t receive or store your payment details.

What We Don’t Do
- We don’t sell or rent information collected through this website.
- We don’t use advertising pixels or remarketing tags, such as the Meta Pixel.
- We don’t add website visitors to an email marketing list.
Children’s Privacy
This website isn’t directed to children under 13, and we don’t knowingly collect personal information from them online. Parents and guardians request appointments for their children.
How We Protect Website Information
Every page on this site uses HTTPS encryption, and only our office staff can read appointment requests. No website or email system can be made completely secure, so please call us if you’d rather not send your information online.

You can always call us instead of using the form. If you sent a request and want us to delete it, call the office and we’ll take care of it.
